Step 1: Pick up a Registration Packet at KMS Registrars' Office in Room F-3.
Step 2: Complete Registration Packet and acquire required documents (listed below).
Step 3: Make an appointment by calling 323-4566 ext. 229 (bring completed Registration Packet and required documents to the appointment).
-Parent/Guardian Photo ID
-Proof of Residence
-Documents from Previous School: Certificate of Release, Transcripts, Withdrawal grades, latest Report Card, Current IEP (Individualized Educational Plan for students with special needs).
-Guardianship Documents (if necessary): If you are not legally listed as the birth parent on the Birth Certificate and the child resides with you, please bring necessary documents. These may include: Power of Attorney, Court Order, Temporary Restraining Orders, and/or Custody Court Order.
-Health Requirements: Tuberculosis Risk Assessment (TB Clearance), Physical Examination and Immunizations (Form 14),
Students must attend the school that serves the geographic area where they reside.
Northern Boundary: South of Lunapule Road (excluding Kona Hillcrest Subdivision) and both sides of Hualalai Road from Queen Kaahumanu Highway to the intersection at Mamalahoa Highway.
Southern Boundary: Mac Farms
Students living outside of the area who wish to attend Konawaena Middle may request an exception from January 1st to March 1st each year by completing a Geographic Exception Request (GE Request Form CHP13-1). Due to limited capacity at KMS, the number of students living outside of our geographic boundaries may be restricted. Transportation to and from school for "GE" students is the responsibility of the parents, unless approved by the district transportation officer.
